For the safety of our students BSD requires each volunteer to have the following clearances on file BEFORE their first day of volunteering with our students! If you do not have clearances on file, you will not be allowed to volunteer at BSD schools.
1. 2023-24 Volunteer Clearance Form-- no cost, every volunteer needs to submit a new form each year.
3. Pennsylvania Criminal Record checks (SP4-164) -- no cost, every volunteer needs to submit a new form every year.
4. Pennsylvania Child Abuse History Clearance (CY113) -- no cost for 1st time, and volunteers need to renew every 5 years. (For renewals before 57 months there is an $8 fee).
5. If you are a new volunteer to BSD you are required to submit the following only once:
A. If you have been a Pennsylvania resident for the entire previous 10 years you may submit the Unpaid Volunteer Affidavit.
B. If you have not been a Pennsylvania resident for the entire previous 10 years you must have a Federal Bureau of Investigations Criminal Background Check - $27.00 (use link under the Department of Education).
